Not to mention that Anarchid banned a romanian a few months ago while i was online, just to be sure it's not me...

There are four kinds of bans (perhaps even more):
1) Account ban.
2) IP ban.
3) Multi-IP-ban, for example a range of numbers. (eg 1.1.1.1 to 1.1.4.255)
4) VPN ban. (similar to number 3 I think)

The first will not affect the provider. The second will only affect one address at any given time, here a banned person getting a new IP means someone else might get the old one. The third will potentially affect all customers of a provider and more. Number four is different in that many VPNs are banned and new players affected by this will need to request for an exception to be made - you might've seen this in a few forum threads.

To answer your question, if anyone is using 2, 3 or 4 then to my limited knowledge they can't make sure that they will not ban innocent people. I wonder how many actual players were lost from these cases combined.
