Difficulty accepting squad invitations during match

I have been trying to squad-up with some friends during a few matches. They will send me a squad invitation, but I am unable to accept it. After receiving the squad invitation, I press TAB and click the commander icon next to the inviting player's name. Instead of accepting their squad invitation, I end up sending a squad invitation in return. What might I be doing wrong?

Did you take a while before accepting the squad invitation? I've noticed that can cause the invitation to "time out" so the button instead sends one rather than accepts one. If not, well, I have no idea.

It makes sense that invites would time out. In this case, however, I am trying to accept the invite pretty much immediately.
